100% Covered Preventive & Diagnostic Services
Your annual membership includes full coverage for:
Comprehensive exam for new patients
Two routine examinations per year
One set of bitewing x-rays per year
Complete series or panoramic x-ray every 3 years
Periapical x-rays

Single Member: $516 per year
Dual Member (parent/child or spouses): $1,020 per year
Each Additional Member: $504 per year
Family plan note: Family members include children enrolled full-time in college until age 23, or children not enrolled full-time in college until age 18.

Program Exclusions
This Dental Savings Program is only honored at Key Dental Group.
The plan cannot be used:
In conjunction with another dental program or special
In conjunction with CareCredit
For services related to injuries covered by worker’s compensation
For treatment that, in the opinion of the treating dentist, falls outside the scope of their capability
For referrals to specialists
For hospitalization or hospital charges of any kind
For dental care costs covered under automobile medical insurance

Before enrolling, please note the following:
There are no refunds for paid benefits, even if the plan is not used during the participation period
The annual savings fee is due at the time of enrollment
The plan must be renewed yearly before it expires
The patient’s portion of treatment is due on the day of service
Members must remain enrolled for the duration of treatment to retain discount program benefits
This program is not dental insurance. It is a reduced-fee savings plan created to help patients receive needed dental care at a more affordable cost.
